Intermediate Care Facilities for People with Intellectual Disability (ICF/ID)

Intermediate Care Facilities (ICF/ID) provide group home living for individuals with developmental disabilities who are in need of a more structured living environment as they acquire skills leading to quality community involvement. Individualized services provided include:
  • Home management skills
  • Recreational/Social activities
  • Community integration training
  • Vocational skills training/Day habilitation
  • Supported employment
  • Nursing services
  • Psychological services
Eligibility for ICF/ID: Adults and children diagnosed with intellectual and developmental disabilities who meet the Level of Care 8 criteria. This means the person must have a valid Determination of Intellectual Disability, or must qualify under an identified related condition with an IQ of 75 or below. Individuals do not have to be in other services with Heart of Texas in order to qualify for placement in our ICF/ID group homes. We have two ICF group homes in Waco, and serve 14 people in the ICF program.

Home and Community Based Services (HCS)

HCS is a Medicaid Waiver program that provides specialized services to people with intellectual and developmental disabilities in their home or in a home-like environment. Individualized services provided include:
  • Residential Support Services/Supervised Living - 3 to 4-bed group homes
  • Foster Care - opportunity for family relationships
  • Personal Attendant Services/Habilitation Services (PAS/HAB) offered as part of Community First Choice
  • Day habilitation - maintaining and/or improving current self-help, grooming, social and other skills
  • Supported Employment - finding and keeping a job
  • Nursing
  • Respite - a relief from caregiver duties
  • Behavioral Supports - an analysis of behaviors and recommendations for developing positive behavior
  • Dental
  • Adaptive aids
  • Minor home modifications

Foster Care Brochure
Eligibility for HCS: Adults and children diagnosed with intellectual and developmental disabilities that meet functional criteria. This means the person must have a valid Determination of Intellectual Disability, or must qualify under an identified related condition with an IQ of 75 or below. Individuals must be offered an HCS slot by the State of Texas and must meet financial criteria for a Medicaid Waiver program.

Similar to HCS, Texas Home Living (TxHmL) is a Medicaid Waiver program that provides specialized services to people with intellectual and developmental disabilities in their home. Individualized services provided include:
  • Personal Attendant Services/Habilitation Services (PAS/HAB) offered as part of Community First Choice
  • Day habilitation - maintaining and/or improving current self-help, grooming, social, and other skills
  • Supported Employment - finding and keeping a job
  • Nursing
  • Respite - a relief from caregiver duties
  • Behavioral Supports - an analysis of behaviors and recommendations for encouraging positive supports
  • Dental
  • Adaptive aids
  • Minor home modifications
Eligibility for TxHmL: Adults and children diagnosed with intellectual and developmental disabilities that meet functional criteria. This means the person must have a valid Determination of Intellectual Disability, or must qualify under an identified related condition with an IQ of 75 or below. Individuals must be currently on the TxHmL Interest List before they will be offered a TxHmL slot by the State of Texas. Criteria for eligibility for TxHmL includes current Medicaid enrollment.
