Psychiatric Medical Services

Offers psychiatric evaluations, medication, mental status monitoring and emergency screenings.

Services Provided:

  • Psychiatric Evaluations
  • Medication Maintenance
  • Crisis Screening, Intervention and Stabilization
  • Inpatient Hospitalization (via contract with DePaul Center)
  • Medication Coordination and Lab Services
  • Nursing Services
  • Family and Consumer Education
Eligibility:
Adults and Children who evidence or are at risk of a severe and persistent mental or emotional disorder. Including persons with dual diagnosis of mental illness and intellectual and developmental disabilities.

Mental Health Outpatient
Therapeutic and counseling services for adults with severe and persistent mental disorders.

Services Provided:

  • Psychosocial Rehabilitation Services
  • Skills Training and Development
  • Crisis Intervention and Brief Therapy
  • Group and Individual Counseling
  • Family Education and Counseling
  • Psychotherapy
  • Discharge Planning

Eligibility:
Adults, 18 and older, who evidence or are at risk of a severe and persistent mental disorder.

Mental Health Case Management
Assist individuals with achieving a quality of life and participation in the community by providing needed services and developing natural support systems.

Services Provided:

  • Assessment
  • Crisis Intervention
  • Monitoring of Psychological Status
  • Planning, Coordination and Monitoring of Provider Services
  • Other services as needed, include:
  • Assertive Community Treatment (Intensive Case Management)
  • Supported Housing
  • Psychosocial Rehabilitation Training
  • Co-occuring Psychiatric and Substance Use Disorder Treatment
  • Patient Family Education Program
  • Skills Training
Eligibility:
Adults with severe and persistent mental disorders with multiple needs that require additional supports to live in the community.

Assists individuals with mental disabilities to choose, obtain and maintain competitive employment.

Services Provided:

  • Skills Training
  • Job Training
  • Job Placement
  • Job Development

Eligibility:
Adults, 18 and older, who evidence or are at risk of a severe and persistent mental disorder.
